If you have been involved in an accident while driving an Exalt Rentals vehicle, please complete this Accident Report as soon as it is safe to do so.

Providing accurate and complete information will assist us in assessing the incident, lodging an insurance claim, arranging repairs, and resolving the matter as quickly as possible.

Please complete all required fields and upload any supporting documentation, including:

• Photographs of all vehicles involved

• Photographs of the accident scene and any damage

• The other driver's licence and contact details

• Registration details of all vehicles involved

• Insurance details of the other driver

• Witness details (if applicable)

• Police Event Number or Incident Number

• Any dashcam footage or additional evidence

If the vehicle is not drivable, or if emergency assistance is required, please contact Exalt Rentals immediately.

Providing false or misleading information may affect your obligations under your Rental Agreement and the assessment of any insurance claim.
